I-trax was incorporated in Delaware on September 15, 2000. I-trax is an integrated health and productivity management company formed by the merger of I-trax, Inc. and Meridian Occupational Healthcare Associates, Inc., which does business as CHD Meridian Healthcare. I-trax acquired WellComm effective February 6, 2002, and also acquired CHD Meridian Healthcare in 2004.
